Casualty Investigation:

Workers Compensation AOE/COE Investigation:

* Recorded or signed statement of employer, applicant(s), and witness(es)
* Index & medical / lost wage authorization(s) from applicant
* Accident scene photos
* I.D. & photos of all relevant parties
* Photo applicant(s) injuries
* Secure employee employment file, medical and/or lost wage reports
